It was due to a rimfire pistol's action being gunked up by that past days shooting (200 rounds +/-) and I had a slam fire occur. I am glad that no one was injured, due to pistol being pointed away from anyone. I cleaned the pistol well and blew it out with canned air and have not had it occur again - that was many years ago.<br /><br />My brother's AD was due to stupidity. He was at my parent's house sitting in living room shortly after a trip to our ranch. Ready to clean it, he pointed the gun at the wall and pulled the trigger to be sure the striker was not pulled back.<br /><br />Only he forgot the MOST IMPORTANT part - ALWAYS check the chamber !<br /><br />So gun goes off and shoots a hole in the wall, angling up to roof. Bullet did not penetrate roof deck, he could not locate a hole. Just stupidity. Dad did not allow him to fix the wall hole on the opposite side of wall (gypsum board) as a reminder. The living room side was panelled in thick wood paneling and the bullet luckily was in one of the wall grooves - you couldn't see it unless you were looking directly at it.
